On 20 May 2025, a FAI into the death in custody at HMP/YOI Polmont of 20 year old Jack McKenzie in September 2021 revealed that prison officers had not conducted adequate checks on him. The Governor, Gerry Michie did not issue any disciplinary action, reprimand, warning or sanction and he did not offer the officers any additional training for which he was criticised in the report, with Sheriff Collins noting that “poor practice must be investigated and sanctioned, not ignored and therefore – apparently – condoned”.
